A travel premium for a non-elective special visit to a patient's home for palliative care, for visits that commence at any time. Fee $37.40 (no max. per time period). Schedule of Benefits Special Visit Premium Table VII, Palliative Care Home Visit (). The time the special visit takes place must be documented in the medical record (). Bill with the appropriate A-prefix assessment from the General Listings.
For fee codes listed in Tables I, II, III, IV, VI, VII, VIII, IX and X the time at which the special visit takes place must be documented on the medical record ()
A travel premium is only eligible for payment for travel from one location to another location ("the destination"). Not eligible for payment when a physician is required to travel from one location to another within the same long-term care facility, hospital complex or within buildings situated on the same hospital campus ()
