Subsequent visit for a hospital in-patient rendered by an Endocrinology & Metabolism specialist during the sixth to thirteenth week of hospitalization inclusive.
Routine assessment in hospital following the hospital admission assessment.
Service must be rendered between the 6th and 13th week of the patient's stay.
Usage limits
Maximum 3 per patient per week; limited to one per patient per day.
