E488 – Repair of labrum

This fee code represents an add-on for the repair of the labrum, and must be billed in addition to R686 (Hip arthroscopy set up, includes when rendered debridement, synovectomy, removal of loose body(ies) and/or screw, drilling of defect, microfracture, abrasion arthroplasty, and/or synovial biopsy). According to the payment rules on page , E487 (Resection of labrum) is not eligible for payment in addition to E488.
When to Use
- Use E488 when performing a formal labral repair (e.g., anchor placement and suture repair) during a hip arthroscopy.
- Use E488 as the add-on code to R686 when the procedure involves restoring the labral seal rather than simple debridement.
Common Pitfalls
- Attempting to bill E487 (resection) and E488 (repair) on the same patient, same day, which will trigger a rejection per N36.
- Billing E488 as a stand-alone code without R686, which is required for the claim to be processed as an add-on.
Billing Tips
- Ensure the operative report explicitly differentiates between labral repair (E488) and labral resection (E487) to justify the selection of the repair code.
