A surgical procedure for synovectomy of the knee for diseased synovium. The procedure can be anterior, posterior, or complete. In accordance with payment rules on page , E492 is not eligible for payment in addition to E494 (Debridement - degenerative cartilage) or E498 (Debridement - trauma).

When to Use

  • Use E492 for the surgical removal of diseased synovium in the knee, such as in cases of chronic inflammatory synovitis or rheumatoid arthritis.
  • Select E492 when the primary clinical objective is the synovectomy itself, rather than the incidental debridement of cartilage.

Common Pitfalls

  • Billing E492 in combination with E494 or E498 will result in a rejection, as these codes are mutually exclusive for the same knee procedure.
  • Attempting to claim E492 alongside debridement codes for the same operative session is a common audit trigger for unbundling.

Billing Tips

  • If an assistant is required for the procedure, you must submit the claim using M400B and provide a letter of justification to a medical consultant for authorization.
  • Ensure the operative report clearly distinguishes the synovectomy from any concurrent cartilage work to justify the selection of E492 over restricted debridement codes.
