When to Use
- Use E627 exclusively as an add-on when performing an emergent endovascular aneurysm repair (EVAR) for a ruptured abdominal aortic aneurysm using code R875.
- Use E627 when performing an emergent endovascular thoracic aneurysm repair (TEVAR) for a ruptured thoracic aortic aneurysm using code R888.
Common Pitfalls
- Claiming E627 with open surgical repair codes (e.g., R802 or R803) will result in automatic rejection, as it is strictly limited to endovascular stent grafting procedures.
- Failing to link E627 to the primary procedure code R875 or R888 on the same claim submission will cause the premium to be denied.
Billing Tips
- Ensure the operative report explicitly documents the rupture to support the medical necessity of the E627 premium in the event of a post-payment audit.
- You may stack after-hours premiums (E409 or E410) on top of the primary procedure fee, but ensure the base procedure is eligible for these premiums before applying them to the total claim.
