E652
E652 – Use of internal mammary, epigastric, or radial artery for bypass graft
OHIP Surgical Assists Code — CARDIOVASCULAR SURGICAL PROCEDURES · Schedule of Benefits
This is an add-on fee payable to the surgeon in addition to the fee for coronary artery repair (R742 or R743) when an internal mammary, epigastric, or radial artery is used for construction of bypass graft. This service is intended to be claimed by the surgeon (suffix A) and is added to the primary surgical fee.
When to Use
- Use E652 when the surgeon harvests and utilizes an internal mammary artery for a coronary bypass graft in conjunction with R742 or R743.
- Use E652 when a radial artery is harvested and used as a conduit during a coronary artery bypass procedure billed under R742 or R743.
- Use E652 when an epigastric artery is utilized as a bypass graft during coronary revascularization procedures.
Common Pitfalls
- Claiming E652 as a standalone procedure; it must be submitted as an add-on to the primary coronary artery repair code (R742 or R743).
- Failing to ensure the surgeon is the primary provider (suffix A) for both the primary procedure and the E652 add-on, as it is not intended for assistants.
- Attempting to bill E652 multiple times for the same patient in a single session; it is a flat fee per procedure regardless of the number of grafts harvested.
Billing Tips
- Ensure E652 is submitted on the same claim as the primary R742 or R743 code to avoid automatic rejection for missing the primary procedure link.
- Verify that the operative report clearly specifies the use of the internal mammary, epigastric, or radial artery to support the E652 claim during potential audits.
