Surgical repair of two coronary arteries. The service fee is for the surgeon's component. Assistant (R743B) and anaesthesia (R743C) services are billed separately based on a unit system. Additional procedures such as the use of an internal mammary, epigastric, or radial artery for the bypass graft (E652), vein patch angioplasty (E646), each additional anastomosis (E654), or performing the procedure off-pump (E645) are billable as add-ons. Payment for a second surgical assistant is allowed without special authorization.
When to Use
- Use R743 for the surgical repair of exactly two coronary arteries; if only one is repaired, use R742 instead.
- Use R743 as the primary procedure code when performing a double coronary artery bypass graft, ensuring all additional grafts are billed separately using E654.
Common Pitfalls
- Failing to switch the assistant and anaesthesia codes to E645B and E645C respectively when performing the procedure off-pump, which will result in a rejection or manual adjustment.
- Incorrectly billing E654 for the first two anastomoses; E654 is only payable for the third and subsequent anastomoses performed during the procedure.
- Neglecting the 50% fee reduction rule for E654 when multiple anastomoses are performed on a single segment of vein (sequential graft).
