Related listings: R743 (two, a separate code); E654 (each additional); E645 off pump coronary artery bypass grafting, to R742 or R743; E651 with coronary endarterectomy and/or gas endarterectomy, to R742 or R743. For the anaesthesiologist, when off-pump coronary artery bypass grafting is rendered, submit the claim using E645C with 40 basic units plus time units instead of R742C or R743C. A second assistant's services are payable without authorization ().

For the surgical assistant, when off-pump coronary artery bypass grafting is rendered, submit claim using E645B with 24 basic units plus time units, instead of R742B or R743B.
Where a single segment of vein is used for more than 2 anastomoses, the second and subsequent anastomoses are to be claimed at 50% of the E654 fee.
