Coronary artery repair for a single vessel. This procedure can be performed with or without a pump (off-pump coronary artery bypass grafting, claimed with add-on code E645). Additional bypasses can be claimed with R743 (for a second bypass) and E654 (for each subsequent bypass). The use of specific arteries like the internal mammary, epigastric, or radial artery can be claimed with add-on code E652. This is a major surgical procedure requiring full anaesthetic and surgical assistant services, with provisions for a second assistant if medically necessary.
When to Use
- Use R742 as the primary code for a single coronary artery bypass graft procedure performed on-pump.
- Use R742 in conjunction with E645 when performing a single coronary artery bypass graft using off-pump (OPCAB) technique.

For the Trauma Premium (E420), the medical record must list the Injury Severity Score (ISS).
For the anaesthesiologist, when off-pump coronary artery bypass grafting is rendered, submit claim using E645C with 40 basic units plus time units, instead of R742C or R743C. See General Preamble .
For the surgical assistant, when off-pump coronary artery bypass grafting is rendered, submit claim using E645B with 24 basic units plus time units, instead of R742B or R743B.
Where a single segment of vein is used for more than 2 anastomoses, the second and subsequent anastomoses are to be claimed at 50% of the E654 fee.
