E761 – Intracorporeal lithotripsy
OHIP Surgical Assists Code — UROGENITAL AND URINARY SURGICAL PROCEDURES · Schedule of Benefits
This is an add-on service for intracorporeal lithotripsy, by any method. It must be claimed in addition to `Z628 (Cystoscopy and diagnostic ureteroscopy - above intramural).
**Parent Procedure Note:** The parent code, Z628, is not eligible for payment with Z638`. This note is located on page of the Schedule.
When to Use
- Use E761 specifically when performing intracorporeal lithotripsy (laser or mechanical) during a ureteroscopy procedure coded as Z628.
- Apply this code when fragmenting ureteral or renal calculi that require active intervention during the diagnostic ureteroscopy session.
Common Pitfalls
- Billing E761 without Z628 will result in an automatic rejection as it is strictly an add-on code.
- Attempting to bill Z638 alongside E761 is a common error; Z638 is explicitly restricted when the parent procedure Z628 is utilized.
- Failure to document the specific method of lithotripsy used can lead to audit scrutiny regarding the necessity of the add-on fee.
Billing Tips
- Ensure Z628 is the primary procedure code on your claim to trigger the eligibility for the E761 add-on fee.
- Verify that no other restricted codes, such as Z638, are included on the same claim, as this will cause a conflict with the parent procedure Z628.
