Urinary catheterization rendered in a hospital setting. This service is only eligible for payment for specific indications: acute retention, change of a Foley catheter or suprapubic tube, or for the instillation of medication. Payment requires that the service be rendered personally by the physician. It is not eligible for payment in conjunction with any surgical procedure or with Z610 (Intravesical instillation).

When to Use

  • Use Z611 when you personally perform an urgent catheterization for a patient presenting with acute urinary retention.
  • Use Z611 for the routine or urgent exchange of an existing Foley or suprapubic catheter, provided you perform the procedure yourself.
  • Use Z611 when performing catheterization specifically to facilitate the instillation of medication, provided you do not also bill Z610.

Common Pitfalls

  • Billing Z611 alongside any surgical procedure code will result in an automatic rejection, as the service is considered bundled into surgical fees.
  • Submitting Z611 when the procedure was performed by a nurse or other staff member will trigger an audit, as the code requires personal physician performance.
  • Attempting to bill Z611 in conjunction with Z610 is prohibited; you must choose the code that best represents the primary intent of the encounter.

Billing Tips

  • Ensure your clinical notes explicitly state that the physician personally performed the catheterization to satisfy the mandatory requirement for payment.
  • If the catheterization is part of a larger surgical procedure, do not bill Z611 separately, as it is deemed included in the surgical fee.
