This service is for catheterization rendered in a patient's home. It is only payable for specific indications, including acute retention, change of a Foley catheter or suprapubic tube, or for the instillation of medication. Per the note on , this service must be rendered personally by the physician and cannot be delegated. It is not eligible for payment in addition to Z610.
When to Use
- Use Z603 for the urgent management of acute urinary retention requiring immediate catheterization in a patient's home.
- Use Z603 for the routine or urgent replacement of an existing Foley or suprapubic catheter when performed personally by the physician in the home setting.
- Use Z603 when performing the therapeutic instillation of medication directly into the bladder via a catheter in a home environment.
Common Pitfalls
- Billing Z603 in addition to Z610 is strictly prohibited; these codes are mutually exclusive for the same service event.
- Delegating the catheterization procedure to a nurse or other staff member will result in a clawback, as the OHIP schedule explicitly requires the physician to render this service personally.
- Billing Z603 for non-indicated reasons, such as routine assessments that do not involve the specific catheter procedures listed in the schedule, will lead to claim rejection.
Billing Tips
- Ensure you append the appropriate Special Visit Premium (e.g., B992 or B994) if the home visit meets the criteria for a non-elective, physician-initiated visit, as Z603 only covers the procedure fee itself.
- Document the specific clinical indication (e.g., 'acute retention' or 'Foley change') clearly in the patient record to justify the use of Z603 over a standard home visit code.
