This is an add-on fee payable in addition to S416 when a gland dissection is performed during a thoraco-abdominal or radical nephrectomy. The primary surgical procedure (S416) must be claimed for this add-on code to be payable.

When to Use

Use E766 exclusively as an add-on when performing a formal lymph node dissection in conjunction with a radical nephrectomy (S416).
Apply this code when the operative report clearly details the systematic removal of regional lymph nodes beyond the standard nephrectomy procedure.

Common Pitfalls

Billing E766 without S416 on the same claim will result in an automatic rejection, as it is strictly an add-on code.
Attempting to use E766 for simple lymph node sampling or incidental node removal during S416 is an audit risk; it requires a formal dissection.
