E767
E767 – Nephrectomy with repair of vena cava for thrombus - below the hepatic vein
OHIP Surgical Assists Code — UROGENITAL AND URINARY SURGICAL PROCEDURES · Schedule of Benefits
This is an add-on fee payable in addition to the fee for S416 (thoraco-abdominal or radical nephrectomy) when the procedure includes the repair of a vena cava thrombus located below the hepatic vein.
When to Use
- Use E767 exclusively as an add-on to S416 when performing a radical nephrectomy that requires the repair of a vena cava thrombus specifically located below the hepatic vein.
- Use this code when the thrombus extent is limited to the infra-hepatic vena cava, distinguishing it from E768 which applies to supra-hepatic involvement.
Common Pitfalls
- Billing E767 without S416 on the same claim will result in an automatic rejection as it is a strictly dependent add-on code.
- Incorrectly billing E767 for thrombus repairs extending above the hepatic vein, which requires the higher-valued E768 instead.
- Failing to document the specific anatomical location of the thrombus (infra-hepatic) in the operative report, which is essential for audit justification.
Billing Tips
- Ensure the operative report explicitly states the thrombus was below the hepatic vein to defend against potential downgrades or audit inquiries regarding the use of E767 versus E768.
