S416 represents a thoraco-abdominal or radical nephrectomy, which is the surgical removal of a kidney. This procedure can be performed laparoscopically, in which case premium E792 is applicable. Additional amounts are payable for specific complexities such as gland dissection (E766) or repair of the vena cava for a thrombus (E767, E768). As per page , the services of a second assistant are payable for this procedure without requiring special authorization.
When to Use
- Use S416 for radical or thoraco-abdominal nephrectomy procedures where the primary intent is the removal of the kidney.
- Use S416 in conjunction with E792 when the nephrectomy is performed via a laparoscopic approach to capture the 25% premium.
- Use S416 as the base code when performing complex resections involving gland dissection (E766) or vena cava thrombus repair (E767 or E768).
Common Pitfalls
- Failing to bill E767 or E768 when a vena cava thrombus is addressed; these are specific add-ons that must be submitted on the same claim as S416.
- Incorrectly assuming a second assistant requires prior approval; S416 explicitly allows for a second assistant without special authorization per GP90.
- Omitting the laparoscopic premium E792 when the procedure is performed laparoscopically, as this is not automatically triggered by the base code.
Billing Tips
- Ensure S416B is used to bill for surgical assistant time units in 15-minute increments, as the base fee for S416 only covers the primary surgeon's procedural fee.
- When performing bilateral procedures or multiple surgeries under the same anaesthetic, ensure the basic units for the assistant are calculated based on the major procedure only.
