E799 – Snare polypectomy of additional polyp (> 1 cm)

This is an add-on service for oesophagoscopy-gastroscopy, with or without duodenoscopy (e.g., Z399, Z400). It is claimed for each additional polyp greater than 1 cm removed via snare polypectomy, subsequent to the removal of the first polyp (claimed as E703). The service is limited to a maximum of two additional polyps per endoscopic procedure.
When to Use
- Use for the second and third snare polypectomy of polyps > 1 cm performed during the same session as the primary snare polypectomy (E703).
- Use only when the polyps are removed via snare technique; do not use for cold biopsy or hot biopsy forceps removal of polyps.
Common Pitfalls
- Claiming E799 for more than two additional polyps, as the code is strictly limited to a maximum of two units per procedure.
- Attempting to claim E799 without a corresponding E703 on the same claim, which will result in an automatic rejection.
- Using E799 for polyps smaller than 1 cm; these should be addressed under the primary procedure code or appropriate biopsy codes.

- Ensure your operative report explicitly documents the size of each polyp removed to justify the use of E799 over standard biopsy codes.
