When to Use

  • Use E814 when performing a formal portal lymphadenectomy during a radical resection for cholangiocarcinoma or gallbladder cancer alongside a base procedure like S291.
  • Apply E814 when conducting a comprehensive lymph node clearance around the porta hepatis in conjunction with major liver resections such as S267, S270, or S271.

Common Pitfalls

  • Billing E814 for simple lymph node sampling or biopsy will result in rejection; the operative report must explicitly document the complete clearance of tissue surrounding the common bile duct, hepatic artery, and portal vein.
  • Submitting E814 without an eligible base procedure (S267, S269, S270, S271, S275, or S291) will trigger an automatic claim rejection.

  • Ensure the operative note specifically lists the dissection of the portal triad structures to satisfy the Ministry's audit requirements for this add-on code.
