An add-on service for the complex closure, repair, and/or reconstruction of surgical defects following procedures like N111, N112, N114 or N116. This service includes duroplasty. It is not payable for simple closures, which are considered part of the primary procedure. See requirements for the specific definition of a 'complex' closure. Not payable with E919.
When to Use
- Use E893 when performing a repair of a CSF leak following procedures like N111 or N114 where the defect extends beyond the sella turcica.
- Use E893 specifically when the reconstruction requires a pedicled vascular flap that measures greater than 3 cm in size.
Common Pitfalls
- Claiming E893 for simple closures or flaps smaller than 3 cm, which are already included in the primary procedure codes N111, N114, or N116.
- Attempting to bill E893 concurrently with E919, as these are mutually exclusive codes.
Billing Tips
- Ensure your operative report explicitly documents the size of the pedicled vascular flap and the anatomical extent of the lesion beyond the sella turcica to support the 'complex' designation.
- Verify that the primary surgical procedure (e.g., N111) is billed alongside E893, as E893 is strictly an add-on service.

A complex closure, repair and/or reconstruction is defined as surgical closure, repair and/or reconstruction: - a. for a lesion extending beyond the sella turcica; and - b. is necessary for repair of CSF leak(s); and - c. requires the use of pedicled vascular flap(s) greater than 3 cm in size.
