Transsphenoidal microscopic resection of lesion(s) originating in the sella turcica requiring simple closure, repair and/or reconstruction of surgical defect(s).
When to Use
- Use N111 for the resection of a pituitary adenoma or other sella turcica lesion via a standard transsphenoidal microscopic approach.
- Use N111 when the procedure is limited to the sella turcica and does not require the extended skull base access defined by E886.
Common Pitfalls
- Billing for separate nasal or sinus procedures (e.g., septoplasty, ethmoidectomy) is prohibited as these are considered included in the E905 add-on or the primary N111 fee.
- Attempting to bill Respiratory or Integumentary surgical codes alongside N111 will result in automatic rejection per the Schedule of Benefits.
- Failure to document the specific anatomical extension of the lesion when claiming the E887 add-on for involvement of the optic chiasm or hypothalamus.
Billing Tips
- Always append E905 to N111 to capture the surgical access fee, as it covers the necessary nasal and sinus work required to reach the sella.
- If the lesion extends beyond the sella, ensure E887 is claimed to reflect the increased complexity of resecting tissue involving the optic apparatus or hypothalamus.
