Endonasal endoscopic resection of non-pituitary lesion(s) not originating from pituitary tissue requiring simple closure, repair and/or reconstruction of surgical defect(s). This procedure is part of the Skull Base Surgery services using an endonasal approach. Add-on codes such as E888, E905, and E886 may be applicable for extended approaches or resections of lesions extending beyond the sella turcica.
When to Use
- Use N116 for the resection of non-pituitary lesions (e.g., craniopharyngiomas or meningiomas) via an endonasal endoscopic approach that necessitates reconstruction of the surgical defect.
- Select N116 when the procedure involves a non-pituitary pathology that does not qualify for the specific pituitary-focused codes like N111 or N114.
Common Pitfalls
- Billing for separate nasal or sinus procedures (e.g., septoplasty or turbinate reduction) is strictly prohibited, as these are considered inclusive to the approach and the E905 add-on.
- Attempting to bill Respiratory or Integumentary surgical codes alongside N116 will trigger an automatic rejection due to the explicit exclusion rules in the Schedule.
- Failure to obtain prior authorization for the surgical assistant (N116B) will result in a payment denial; ensure the required letter is submitted to the medical consultant.
Billing Tips
- Always append E905 to N116 to capture the surgical access fee, which covers the endoscopic approach and associated nasal/sinus work.
- If the lesion extends to the optic nerve, chiasm, or hypothalamus, ensure E888 is billed on the same claim to capture the additional $500.00 flat fee.
