This is an add-on fee payable in addition to the fee for the primary procedure, either N122 (Endovascular repair of intracranial aneurysm, carotid circulation) or N125 (Endovascular repair of intracranial aneurysm, vertebrobasilar circulation), when the treated aneurysm is greater than 2.5 cm in diameter.
When to Use
- Use E894 as an add-on when performing an endovascular repair of a carotid circulation aneurysm (N122) where the pre-operative imaging confirms a diameter exceeding 2.5 cm.
- Apply E894 in conjunction with N125 when treating a vertebrobasilar circulation aneurysm that measures greater than 2.5 cm in its greatest dimension.
Common Pitfalls
- Billing E894 without explicitly documenting the aneurysm size in the operative report, which leads to automatic rejection or clawbacks during audits.
- Attempting to claim E894 for aneurysms measuring exactly 2.5 cm; the code strictly requires the diameter to be greater than 2.5 cm.
