When to Use
- Use N122 for the endovascular treatment of aneurysms located in the anterior circulation, such as the internal carotid, anterior cerebral, or middle cerebral arteries.
- Select N122 when performing comprehensive management including catheterization and angiography for anterior circulation pathology, as these are bundled into the fee.
Common Pitfalls
- Billing for diagnostic angiography or image guidance separately is a common rejection, as these services are explicitly included in the N122 fee.
- Attempting to bill E991 for liquid embolic agents or standard coil embolization will result in a rejection, as E991 is strictly reserved for flow-diverter or intra-saccular device deployment.
- Submitting multiple claims for E991 when using multiple devices in the same operative session is incorrect; it is payable only once per session.
Billing Tips
- Always append E991 to your N122 claim when deploying a flow-diverter or intra-saccular device to capture the additional $700 fee.
- Ensure no other N-prefix services are billed for the same operative session, as N122 is mutually exclusive with all other N-series codes except N125.
