This code represents an additional fee for mastoid cavity obliteration. It must be claimed in addition to the primary procedure: E315 (Revision mastoidectomy), E320 (Cortical mastoidectomy), or E322 (Modified or radical mastoidectomy).

When to Use

  • Use E946 when performing obliteration of the mastoid cavity using autologous tissue, bone chips, or synthetic materials during a primary or revision mastoidectomy.
  • Claim E946 in conjunction with E322 when the mastoid cavity is obliterated to prevent chronic drainage or to facilitate a canal-wall-up conversion.

Common Pitfalls

  • Submitting E946 as a standalone code without an associated primary mastoidectomy code (E315, E320, or E322) will result in an automatic rejection.
  • Billing E946 for simple closure or standard wound management that does not involve formal obliteration of the mastoid cavity is considered an inappropriate use of the code.
