A modified or radical mastoidectomy. This is an excision procedure of the middle ear. Add-on codes are available for concomitant procedures: - E946: with mastoid cavity obliteration - E959: with meatoplasty and/or canalplasty - E960: with ossiculoplasty - E985: with tympanoplasty This procedure is listed under the Ocular and Aural Surgical Procedures section of the Schedule of Benefits.
When to Use
- Use E322 when performing a modified or radical mastoidectomy to address chronic suppurative otitis media or cholesteatoma that cannot be managed by simpler procedures like E315 (simple mastoidectomy).
- Select E322 when the surgical objective is the complete removal of disease from the mastoid air cells and the middle ear space, specifically involving the creation of a common cavity.

- Billing E322 in conjunction with E315 is a common error; E315 is considered a component of the more extensive E322 procedure and is not separately payable.
