E315 – Revision mastoidectomy with revision of middle ear
OHIP Surgical Assists Code — Y. Ocular and Aural Surgical Procedures · Schedule of Benefits
Performs a revision mastoidectomy, a surgical procedure on the mastoid process behind the ear, which also includes a revision of the middle ear. This service is eligible for surgical assistant (suffix B) and anaesthesia (suffix C) services, with specified base units for each (6 for assistant, 10 for anaesthesiologist). Additional procedures may be billed as add-ons, including E946 for mastoid cavity obliteration, E959 for meatoplasty and/or canalplasty, E960 for ossiculoplasty, and E985 for tympanoplasty.
When to Use
- Use E315 when performing a revision mastoidectomy that necessitates concurrent surgical intervention within the middle ear space.
- Select E315 instead of E320 or E322 when the procedure specifically involves re-operating on a previously mastoidectomized ear to address recurrent disease or structural failure.
Common Pitfalls
- Billing E315 in conjunction with E320 or E322 is considered unbundling, as E315 is the comprehensive code for the revision of both the mastoid and middle ear.
- Failing to document the specific 'revision' nature of the procedure in the operative report will lead to audit rejections, as primary mastoidectomy codes must be used for initial surgeries.
Billing Tips
- Always append the appropriate add-on codes like E960 for ossiculoplasty or E985 for tympanoplasty if performed during the same session, as these are distinct procedural components not included in the E315 base fee.
