F009 – Metacarpal - closed reduction

A surgical procedure for the management of a metacarpal fracture via closed reduction. The service is eligible for anaesthesia with 6 base units. An additional fee (E584) can be claimed for applying a plaster cast outside of a hospital. If more than one metacarpal is reduced, E504 can be added for each additional fracture.
When to Use
- Use F009 for the closed reduction of a single metacarpal shaft or neck fracture when no surgical incision is required.
- Use F009 in conjunction with E504 when you perform closed reductions on two or more separate metacarpals during the same operative session.
Common Pitfalls
- Billing F009 alongside F008 or F011 will trigger an automatic rejection as these codes are mutually exclusive for the same anatomical site.
- Attempting to bill an assistant fee using a 'B' suffix will result in a rejection; you must submit M400B with a formal letter of justification to the Ministry for review.
- Claiming E584 for a cast applied within a hospital setting is an audit risk, as this add-on is strictly reserved for procedures performed outside of a hospital facility.

- Ensure the fracture site is clearly specified in your claim to avoid confusion with F008 (Phalangeal fracture) or F011 (Carpal fracture) during manual audits.
- Always append E504 for each additional metacarpal reduced beyond the first to ensure you are compensated for the increased complexity of the procedure.
