F008 – Metacarpal fracture - no reduction, rigid immobilization

This service covers the management of one or more metacarpal fractures where no reduction is necessary. The treatment involves the application of rigid immobilization. An additional fee (E584) may be claimed for the application of a plaster cast outside of a hospital setting. This code is for one or more metacarpals.
When to Use
- Use F008 for stable, non-displaced metacarpal fractures where the clinical decision is to immobilize without attempting a reduction maneuver.
- Use this code for multiple metacarpal fractures if no reduction is performed on any of the involved bones, as the code covers one or more metacarpals.
Common Pitfalls
- Do not bill F008 if a reduction maneuver was performed; use F009 instead for metacarpal fractures requiring reduction.
- Avoid billing F008 in conjunction with an office visit code (A-prefix) for the same encounter, as the surgical fee is intended to be comprehensive of the assessment and immobilization.
- Do not attempt to bill F008 multiple times for multiple metacarpal fractures on the same hand; the code is defined as 'one or more' and is not additive.
Billing Tips
- If the immobilization is performed in your office, ensure you append E584 to the claim to capture the additional fee for the plaster cast application.
- If the patient is under 16 years of age, remember to apply the appropriate age-based surgical premium to the F008 base fee to maximize your claim.
