This code represents the technical component for using a contrast agent with an echocardiography study. It is an add-on to be claimed with either a complete study (G570) or a stress study (G582). Its use is indicated for difficult-to-image patients where standard imaging is suboptimal.

When to Use

  • Use G585 when performing a complete echocardiogram (G570) where at least two contiguous segments of the left ventricle are not adequately visualized on non-contrast images.
  • Apply this code during a stress echocardiogram (G582) when contrast is medically necessary to improve endocardial border definition in patients with suboptimal acoustic windows.

Common Pitfalls

  • Claiming G585 without a corresponding professional component (G571 or G583) will result in an automatic rejection.
  • Billing G585 for routine studies without documenting the specific failure to visualize two or more contiguous segments on the non-contrast portion of the exam creates a significant audit risk.
  • Failing to apply the 86.10% reduction when billing G585A in a hospital setting will lead to overpayment recovery.

The physician performing the service must be able to establish they meet the required training levels (Level III, or Level II plus additional training, or specific grandfathering criteria).

The use of a contrast agent must be justified by difficult imaging where two or more contiguous segments are not seen on recent non-contrast images.

G585 is not eligible for payment unless a permanent record of study images and loops is maintained on an appropriate dynamic medium, either videotape or digitally.
