G875 – Botulinum toxin injection - first injection
OHIP Diagnostic & Therapeutic Procedures Code — DIAGNOSTIC AND THERAPEUTIC PROCEDURES · Schedule of Benefits
This service is for the first botulinum toxin injection for the treatment of Oromandibular dystonia, limb dystonia, cervical dystonia, or spasticity. Additional injections administered during the same session should be claimed using G876.
When to Use
- Use G875 as the primary code for the first botulinum toxin injection site when treating confirmed cervical dystonia, limb dystonia, or spasticity.
- Use G875 when performing the initial injection of a multi-site treatment session, ensuring all subsequent injection sites are billed using G876.
Common Pitfalls
- Billing G875 multiple times for the same patient on the same day; only one G875 is permitted, with all additional sites captured under G876.
- Attempting to bill separate EMG or ultrasound guidance codes (other than G877 or G878) on the same day, which will trigger an automatic rejection.
- Failing to include the appropriate guidance code (G877 or G878) when EMG is utilized, as G875 does not inherently include the professional fee for guidance.
Billing Tips
- Always pair G875 with G876 to capture the full volume of injections, up to the maximum of 11 additional units per session.
- If performing the procedure in an office setting, remember to add E543 when using a disposable EMG electrode alongside G877 or G878 to recover supply costs.
