G877 – EMG and/or ultrasound guidance for Botulinum toxin injection (one injection)

This service is for the use of Electromyography (EMG) guidance for one Botulinum toxin injection, when such guidance is required to determine the correct injection site. It is an add-on service and must be billed in conjunction with one of the following primary procedures: G870, G873, G874, or G875. If EMG guidance is used for two or more injections, G878 should be claimed instead of G877. An additional fee for a disposable EMG hypodermic electrode (E543) can be claimed when this service is performed outside of a hospital.
When to Use
- Use G877 when you perform a single Botulinum toxin injection using EMG or ultrasound guidance to localize a specific muscle, such as for focal dystonia or spasticity.
- Select G877 when the guidance is required for only one injection site; if you perform guidance for two or more sites, you must switch to G878.
Common Pitfalls
- Billing G877 as a standalone service will result in rejection; it must be submitted on the same claim as a primary injection code like G870, G873, G874, or G875.
- Claiming G877 and G878 on the same date of service is a common audit trigger as they are mutually exclusive; choose the code that reflects the total number of guided injections performed.
Billing Tips
- If performing the procedure in your office, remember to add E543 to your claim to recover the cost of the disposable EMG hypodermic electrode, as this is not included in the G877 fee.
