When to Use
- Use G874 specifically for the treatment of sialorrhea (excessive drooling) or epiphora (excessive tearing) via botulinum toxin injection.
- Select G874 when the procedure is performed unilaterally or bilaterally, as the fee is inclusive of both scenarios.
Common Pitfalls
- Billing G874 alongside general diagnostic EMG or ultrasound codes is prohibited; only G877 or G878 are permitted for guidance.
- Attempting to bill G874 with other botulinum toxin codes (e.g., G870 or G873) for the same patient encounter may trigger a rejection if the clinical documentation does not clearly distinguish separate anatomical sites or indications.
Billing Tips
- Always append G878 if you are performing two or more injections with EMG guidance to ensure you receive the higher add-on fee rather than the single-injection G877.
