J315 – Graded exercise to maximum tolerance

J315 is a Stage I graded exercise test to maximum tolerance. The service requires continuous monitoring of heart rate, oximetry, and ventilation at rest and at each workload.
This service has two components:
• Technical Component (H Fee): The fee payable to the facility for the technical part of the service. As per of the Schedule, the technical component is not eligible for payment if the service is rendered to a hospital in-patient or an out-patient who is admitted within 24 hours for the same condition. The technical component is also not payable if performed outside a hospital.
• Professional Component (P Fee): The fee payable to the physician for their professional service (e.g., interpretation and report).
Referrals from a Nurse Practitioner for this service are insured when performed in an ICHSC or hospital, as per .
