J866 is an add-on fee payable for the application of Single-Photon Emission Computed Tomography (SPECT). As per the Schedule, it can be added to nuclear medicine examinations, with the specific exceptions of J807, J808, or J852. It is limited to a maximum of one per examination. The fee for this service is added to the fee for the primary nuclear medicine procedure.
When to Use
- Use J866 as an add-on to standard nuclear medicine imaging procedures like bone scans (J809) when SPECT acquisition is performed to improve diagnostic sensitivity.
- Apply J866 when performing tomographic reconstruction for myocardial perfusion imaging or other organ-specific studies that permit SPECT enhancement.
Common Pitfalls
- Billing J866 alongside J807, J808, or J852 will result in an automatic rejection as these codes are explicitly excluded in the Schedule of Benefits.
- Submitting J866 more than once per examination is a common audit trigger, as it is strictly limited to a maximum of one per procedure.
Billing Tips
- Ensure J866 is appended to the primary nuclear medicine procedure code on the same claim to avoid processing errors.
- If performing hybrid imaging, consider whether J903 is more appropriate, as J866 is strictly for SPECT and does not account for the CT component of hybrid tomography.
