This service is for the completion of the amalgamated Health Status Report and Activities of Daily Living Index forms for a patient's initial application to the Ontario Disability Support Program (ODSP). This service is not an 'insured service' within the meaning of the Health Insurance Act but is paid by the Ministry of Health, acting as paying agent on behalf of the Ministry of Children, Community and Social Services (MCCSS), the Ministry of the Attorney General, the Ministry of the Solicitor General, and the Workplace Safety and Insurance Board (WSIB).
When to Use
- Use K050 exclusively for the initial completion of the amalgamated Health Status Report and Activities of Daily Living Index for a patient's first-time ODSP application.
- Use this code when the patient presents specifically for the assessment and documentation required to initiate the ODSP disability determination process.
Common Pitfalls
- Billing K050 for subsequent ODSP updates or reviews, which should instead be billed under K051 or other relevant Appendix F codes.
- Attempting to bill an office visit code (A007) in addition to K050 on the same day, which is considered double-billing for the same service encounter.
- Submitting K050 for non-ODSP disability forms, such as private insurance or CPP disability, which are not covered under this specific fee.
Billing Tips
- Ensure the claim is submitted with the correct diagnostic code related to the patient's primary disabling condition to avoid processing delays.
