This service is for the completion of the Activities of Daily Living Index form when it is filled out separately as part of an initial application for the Ontario Disability Support Program (ODSP). As per , this service is not an 'insured service' under the Health Insurance Act, but is paid by the Ministry of Health on behalf of the Ministry of Children, Community and Social Services (MCCSS).
When to Use
- Use K052 when you are completing the Activities of Daily Living Index as a standalone document for an initial ODSP application.
- Use this code when the Health Status Report (K051) has been completed separately and you are now specifically addressing the functional assessment portion of the application.
Common Pitfalls
- Claiming K052 in conjunction with K050 will result in an automatic rejection, as K050 is the amalgamated form that already includes the ADL index.
- Submitting K052 for a patient who is already receiving ODSP benefits is an error, as this code is strictly reserved for the initial application process.
