K051 – Health Status Report for initial ODSP application

The service is for the completion of a Health Status Report form as part of an initial application for the Ontario Disability Support Program (ODSP). This fee applies when the form is completed separately from the Activities of Daily Living Index form (K052). The amalgamated form is billable as K050. As per , this service is not an 'insured service' under the Health Insurance Act but is paid by the Ministry of Health, acting as a paying agent on behalf of the Ministry of Community and Social Services (MCSS).
When to Use
- Use K051 when you are completing only the Health Status Report portion of the ODSP application package for a patient.
- Select K051 if the patient's application does not require the Activities of Daily Living Index (K052) at the time of the initial assessment.
Common Pitfalls
- Billing K051 in conjunction with K052 will result in a rejection; use K050 instead if both forms are completed for the same patient.
- Submitting K051 for subsequent reviews or updates to an existing ODSP file is incorrect, as this code is strictly for the initial application process.

- Ensure the patient's health card number is correctly linked to the ODSP application file, as K051 is processed as a non-insured service paid by the Ministry.
