The service rendered by the most responsible physician for completion and submission of a home care referral form to Ontario Health atHome on behalf of a patient for whom the physician provides on-going medical care. The amount payable for this service is as shown and is in addition to the assessment fee payable, where applicable. The amount payable for completion of the home care referral form if completed in whole or in part by a person other than the physician or the physician’s employee is nil.
When to Use
- Use K070 when initiating a new home care referral for a community-based patient who is not currently receiving Ontario Health atHome services.
- Claim K070 in addition to an office visit (e.g., A007) when the referral form is completed by you or your staff during the encounter.
Common Pitfalls
- Do not bill K070 if the patient is already an active home care client, as it will be rejected; use K071 or K072 for ongoing management or reassessment instead.
- Avoid billing K070 for patients in long-term care facilities, as this service is already included in the W010 monthly management fee.
Billing Tips
- Ensure the referral form is completed by you or your direct employee; claims will be rejected if the form is completed entirely by external agencies or hospital staff.
