  • Billing L067 for specimens collected during a hospital encounter or when the patient is admitted within 24 hours, which triggers an automatic rejection under GP11 rules.
  • Attempting to bill L067 alongside a surgical pathology professional component (L861-L867) without ensuring the technical procurement is distinct and documented as a separate service.
