Interpretation of the results of the laboratory procedure.
Providing a written interpretative report of the procedure to the referring provider, if other than the interpreting physician.
Providing premises, equipment, supplies and personnel for any aspect(s) of the constituent elements that is (are) performed at a place other than the place in which the laboratory procedure is performed.
Laboratory services are only eligible for payment if the result of the test(s), the physician’s interpretation of the results of the test(s) and the treatment decision based on the test results are documented in the patient’s permanent medical record.
