When to Use
- Use L494 for the collection of a venous blood specimen by venipuncture when performed as a standalone service.
- Bill this code when a patient presents specifically for blood work ordered by another physician or for routine monitoring where no other assessment code is claimed.
Common Pitfalls
- Do not bill L494 in addition to an office visit code (e.g., A007) as the venipuncture is considered bundled into the assessment fee.
- Avoid billing L494 if the blood collection is performed by nursing or support staff in a facility where the physician is already claiming a global or facility-based fee that includes laboratory support.
