When to Use
- Use L570 for the excision of a single benign skin lesion, such as a skin tag or small seborrheic keratosis, that does not require formal closure or complex reconstruction.
- Select L570 when performing a simple shave or snip excision of a superficial lesion where the primary intent is removal rather than diagnostic biopsy (which would be billed under Z300).
- Apply L570 for the removal of a single lesion when the procedure is performed independently and not as part of a more extensive surgical excision billed under the S-series codes.
Common Pitfalls
- Billing L570 in addition to a consultation or assessment code (A007/A001) on the same day is often rejected unless the lesion removal was an urgent, unplanned procedure distinct from the primary visit reason.
- Attempting to bill L570 multiple times for multiple lesions on the same day will trigger a rejection; use the appropriate multiple-lesion modifiers or secondary codes if applicable.
- Confusing L570 with Z300; L570 is for therapeutic removal, whereas Z300 is strictly for diagnostic tissue sampling.
