When to Use
- Use L625 for the collection of a venous blood sample by venipuncture when performed as a standalone service.
- Bill this code when a patient presents specifically for blood work ordered by another physician or for routine monitoring where no other assessment code is claimed.
Common Pitfalls
- Do not bill L625 in addition to an office visit code (e.g., A007) as the venipuncture is considered bundled into the assessment fee.
- Avoid billing L625 if the blood collection is performed by a nurse or lab technician in a facility where the physician does not personally perform the venipuncture.
- L625 is frequently rejected if billed on the same day as a minor procedure or diagnostic test that already includes the cost of specimen collection in its schedule of benefits.
