When to Use
- Use L650 for the removal of a single benign skin lesion, such as a skin tag or small seborrheic keratosis, when performed via simple excision or curettage.
- Select L650 when the procedure is performed as a standalone service, as it cannot be claimed in addition to a minor assessment (A007) or a general assessment (A001) on the same day.
Common Pitfalls
- Billing L650 alongside an office visit code like A007 will result in a rejection for 'duplicate service' or 'included in another service'.
- Attempting to claim L650 for multiple lesions on the same day is restricted; you must use the appropriate multiple-lesion modifiers or specific codes if the total count exceeds the allowable limit for a single encounter.
Billing Tips
- If you are performing a diagnostic biopsy on the same lesion, you must choose between L650 and the biopsy code (e.g., Z402), as you cannot bill both for the same anatomical site.
