When to Use
- Use L675 for the excision of a malignant lesion on the face, ears, or neck where the diameter of the lesion is 0.5 cm to 1.9 cm.
- Select L675 instead of L674 when the lesion is located in a high-visibility area, as L675 carries a higher fee for facial/neck sites compared to trunk or extremity sites.
Common Pitfalls
- Billing L675 for a benign lesion excision, which must be billed under the G-series codes (e.g., G381) rather than the L-series malignant excision codes.
- Attempting to add a tray fee or minor procedure premium to L675, as the fee is inclusive of the procedure and standard supplies.
Billing Tips
- Always document the exact size of the lesion in centimeters, as the Ministry may audit L675 claims if the size is not clearly stated to justify the code selection over smaller or larger excision codes.
