When to Use
- Use L676 for the excision of a malignant lesion on the face, ears, or neck where the diameter of the lesion is 0.5 cm to 1.9 cm.
- Select this code when performing a full-thickness excision of a basal cell or squamous cell carcinoma that requires primary closure, as opposed to a simple biopsy (E400 series).
Common Pitfalls
- Billing L676 in conjunction with a minor procedure code like G372 for the same lesion is considered unbundling and will trigger an automatic rejection.
- Failing to document the exact size of the lesion in the chart notes is a primary cause for clawbacks during OHIP audits, as the code is size-dependent.
Billing Tips
- If the excision requires a complex reconstruction or flap, bill the appropriate code from the S-series instead of L676 to reflect the increased complexity and higher fee.
