When to Use
- Use L733 for the excision of a single benign skin lesion, such as a seborrheic keratosis or skin tag, when the procedure is performed via simple excision.
- Select L733 when performing a diagnostic biopsy of a suspicious lesion that does not meet the criteria for a more complex excision code like Z302 or Z303.
Common Pitfalls
- Billing L733 in addition to a minor assessment code (A007) for the same patient on the same day is often rejected unless the procedure is medically necessary and distinct from the assessment.
- Attempting to bill L733 for multiple lesions on the same site will trigger a rejection; use the appropriate multiple procedure rules or specific excision codes if the total size exceeds the threshold for a single L733.
Billing Tips
- Ensure the pathology report or clinical notes explicitly document the benign nature of the lesion to support the use of L733 over higher-valued excision codes.
- If performing a biopsy and an excision on the same lesion, you cannot bill both; bill only the most comprehensive procedure code.
