M145 – Wedge resection of lung

A surgical procedure involving the excision of a wedge-shaped section of lung tissue. As a surgical service, the fee for `M145` includes pre-operative care (visits 1-2 days before surgery), the operation itself, and post-operative care for up to two weeks, with the exception of the first and second post-operative visits and the day of discharge visit which may be claimed separately. When to Use
- Use M145 for the excision of a peripheral lung nodule or wedge-shaped segment of pulmonary parenchyma when a formal lobectomy is not indicated.
- Use M145 when performing a diagnostic wedge resection to obtain tissue for definitive pathological diagnosis of indeterminate lung lesions.
Common Pitfalls
- Billing M145 alongside a thoracoscopic biopsy code (e.g., M149) is often rejected as the wedge resection inherently includes the biopsy of the tissue removed.
- Failing to include the E683 premium when the wedge is performed via VATS or robotic approach results in a significant underpayment of the procedure fee.
- Attempting to bill routine post-operative visits within the 14-day global period, forgetting that only the first and second post-operative visits and the day of discharge visit are exempt from the global fee.
Billing Tips
- Ensure the E683 premium is appended to the M145 claim when using minimally invasive techniques, as it provides a 35% increase to the base surgical fee.
- If the wedge resection is performed for trauma, ensure the ISS score is clearly documented in the chart to support the E420 trauma premium.
