When to Use

  • Use N295 for the surgical excision of a symptomatic Morton's neuroma in the intermetatarsal space.
  • Use N295 for the excision of a subcutaneous glomus tumour or a small benign cutaneous nerve tumour (e.g., schwannoma) where the primary intent is the removal of the nerve-associated mass.

Common Pitfalls

  • Do not bill N295 for simple skin lesion excisions; the pathology must confirm a neuroma or nerve tumour to withstand an audit.
  • Avoid billing N295 in conjunction with N286 for the same anatomical site, as these are typically considered mutually exclusive or bundled procedures.
  • Failure to document the use of an operating microscope will lead to the rejection of the 40% premium code E906.

Billing Tips

  • Always append E911 if you perform the additional step of burying the proximal nerve stump into bone or muscle to prevent recurrence.
  • Ensure the operative report explicitly mentions the use of an operating microscope to justify the E906 premium.
