Performs a rotation, transposition, or Z-plasty for a defect that is less than 2 cm in average diameter. This service is for locations on the body excluding the face, neck, or scalp. As per the note on page of the Schedule, this procedure includes undermining, with the extent depending on the specific site and size of the defect.
When to Use
- Use R072 when closing a surgical defect on the trunk or extremities using a rotation or transposition flap where the average diameter is less than 2 cm.
- Select R072 instead of simple excision codes (e.g., S060-S063) when the defect requires a Z-plasty to release tension or redirect a scar line on the body.
Common Pitfalls
- Billing R072 in addition to an excision code (e.g., S060) for the same lesion is a common rejection; the flap code is intended to include the excision of the lesion itself.
- Applying R072 to facial or scalp defects is an audit risk, as these locations require specific reconstructive codes (e.g., R075 or R076) rather than the general body codes.
Billing Tips
- If performing an excision for malignancy requiring frozen section control, ensure you add E540 to R072 to capture the 25% premium allowed under the M16 section rules.
