A surgical procedure involving rotations, transpositions, or Z-plasties to repair a skin defect with an average diameter between 2.1 cm and 5 cm. This code applies to procedures performed on areas of the body other than the face, neck, or scalp. The service includes undermining of the skin flap as noted on page :. The amount payable depends on the size and location of the area grafted and the type of graft. Additional procedures other than the skin grafting (e.g., tendon grafts, inlay grafts) are payable in addition to the skin flap or graft. For sentinel node biopsy, refer to Z427.
When to Use
- Use R075 for Z-plasty, transposition, or rotation flap repairs on the trunk or extremities when the defect diameter is between 2.1 cm and 5 cm.
- Select R075 when the procedure involves significant undermining to mobilize local tissue to cover a defect, distinguishing it from simple primary closure codes.
Common Pitfalls
- Do not use R075 for procedures on the face, neck, or scalp; these require specific facial flap codes in the R040-R049 range.
- Avoid billing R075 for simple excisions or primary closures; the code specifically requires the technical complexity of a rotation, transposition, or Z-plasty flap.
