Complex fascial reconstruction of the femur, with or without a synthetic graft or rotation flap. According to the definition on page of the Schedule, when 'with or without' is used in a service description, the other services or elements mentioned are optional. However, if they are performed, they are considered part of the insured service and are included in the fee for the main procedure.
